Tier 1 Care Navigator
Department: Care Coordination
The Care Navigator plays a key role in delivering high-quality, patient-centered care coordination services for 1 True Health customers. This position ensures patients receive effective support across Remote Physiologic Monitoring (RPM), Chronic Care Management (CCM), Principal Care Management (PCM), Transitional Care Management (TCM), and other care delivery programs. The Care Navigator is responsible for guiding patients through their health and wellness journeys, maintaining accurate documentation, educating patients, and supporting comprehensive care planning.
Care Navigators help patients understand their care plans, stay engaged with their health goals, and successfully participate in digital health and chronic care programs. This role is critical in meeting departmental and corporate performance targets, including clinical quality, patient engagement, and billing accuracy.
This is a part-time, in-home/virtual position with the opportunity to become full-time depending on skill, will, and demand. Tier 1 Care Navigators are guaranteed 20 hours per week at $20/hour. All part-time personnel are provided flexibility around other obligations but will require predetermined work blocks, primarily weekday days.
Key Responsibilities
Care Coordination and Program Enrollment
- Oversee patient onboarding, enrollment, and service delivery for all care management programs, including RPM, CCM, PCM, TCM, and Evaluation & Management-related support.
- Serve as the primary point of contact for assigned patients, building rapport and fostering trust.
Comprehensive Care Planning
- Implement and manage individualized Comprehensive Care Plans for all eligible patients.
- Monitor patient progress, barriers, health conditions, and needed interventions.
Care Documentation
- Utilize internal documentation systems accurately and consistently to record encounters, assessments, interventions, and time-based billing requirements.
- Ensure documentation supports compliant and timely billing for clinical services.
Patient Engagement and Education
- Educate patients on their conditions, devices, and care protocols using established patient engagement and patient education pathways.
- Provide consistent ongoing encouragement and support to optimize patient compliance and outcomes.
Health and Wellness Program Development
- Deliver 1 True Health’s health and wellness program content to patients to support disease prevention, health optimization, and long-term chronic condition management.
Quality, Compliance and Performance
- Maintain accuracy in all clinical service delivery and time-based code attainment.
- Meet or exceed individual productivity, engagement, and billing goals as outlined by department leadership.
- Adhere to all internal policies, privacy requirements, and regulatory standards.
Qualifications
Required
- Experience in care coordination, patient support, or related healthcare roles.
- Strong communication skills (verbal and written).
- Ability to document clearly and follow structured workflows.
- Comfort using digital health platforms and patient-facing technology.
- Access to high-speed internet access and computer.
Preferred
- Background in chronic care management, health education, or care navigation.
- Familiarity with RPM, CCM, PCM, or TCM program requirements.
- Experience in telehealth or digital health environments.
